Complete and submit the Consolidus Statement of Intent form. A Consolidus representative will contact members in 2-3 business days to discuss needs, define the scope and conduct product demo. Contact your E&I Representative at any time for assistance.
Supplier Direct
When ready to move forward, members will sign a Consolidus Supplemental Agreement (with commercial terms) to initiate services. Your E&I Representative will also follow up to ensure successful enrollment.
