Submit the Smile Promotions Alignment Authorization form. Upon receipt of the AAF, the account manager will contact the member in 1-2 business days to discuss needs and prepare a tailored price proposal. Contact your E&I Representative at any time for assistance.

When ready to move forward and begin placing orders, Smile Promotions will be created or activate the member account in Smile Promotions system. E&I will also follow up to ensure successful enrollment.
